Welcome to FAMS Health Office! 
 
The health office is open to all students and staff members at FAMS. The goal of our health office is to promote optimal health, physically, emotionally, and socially while providing a safe and caring environment. Student and staff wellness is paramount!
 
As former US Surgeon General Jocelyn Elders, MD, once said, "You cannot educate a child who is not healthy, and you cannot keep a child healthy who is not educated."
 
The FAMS health office always strives to maintain overall wellness. Wellness does not just matter - it is critical.  It is critical not just for the student, but for teachers and staff!
 
Parents and guardians are encouraged to contact the health office with any questions or concerns throughout the year. Welcome to the FAMily.

Requirements for all 6th grade incoming and transfer students from out of state or country 
 
All 6th grade students must provide an up to date physical (completed with in last 365 days) and proof of immunizations PRIOR to the first day of school.
 
The Tdap and meningoccal vaccines are required by the State of New Jersey for all students entering the 6th grade. Students will be excluded from school for failure to provide these records. 
 
Please have your child's physician complete the "Medical Eligibility Form" for all physicals. All medical forms can be found under the Health Concerns and Medical Forms tab.
 
Transfer students from out of country :
Students transferring from a high Tuberculosis (TB) incidence country will need to be screened for TB prior to school admission. An interferon gamma release assay (IGRA) blood test or a Mantoux tuberculin skin test is acceptable for TB screening into school.
