Welcome to FAMS Health Office!
The health office is open to all students and staff members at FAMS. The goal of our health office is to promote optimal health, physically, emotionally, and socially while providing a safe and caring environment. Student and staff wellness is paramount!
As former US Surgeon General Jocelyn Elders, MD, once said, "You cannot educate a child who is not healthy, and you cannot keep a child healthy who is not educated."
The FAMS health office always strives to maintain overall wellness. Wellness does not just matter - it is critical. It is critical not just for the student, but for teachers and staff as well!
Parents and guardians are encouraged to contact the health office with any questions or concerns throughout the year. Welcome to the FAMily.

Incoming 6th grade and transfers from out of state or country: See forms below
ALL 6th graders must provide proof of : Tdap and meningoccal vaccines.
These are State of New Jersey requirements without them you may be excluded from the school building. The Immunization record along with a current physical should be provided to the health office prior to the first day of school.
Please have your child's physician complete the "Preparticipation Physical Evaluation (PPE)" form found below for all physicals.
Transfer students from out of country:
Students transferring into district from a high Tuberculosis (TB) incidence country will need to be screened for TB prior to school admission. An interferon gamma release assay (IGRA) blood test or a Mantoux tuberculin skin test is acceptable for TB screening into school.
