FAMS Health Office/Incoming Student Information

Welcome to FAMS Health Office! 
 
The health office is open to all students and staff members at FAMS. The goal of our health office is to promote optimal health, physically, emotionally, and socially while providing a safe and caring environment. Student and staff wellness is paramount!
 
As former US Surgeon General Jocelyn Elders, MD, once said, "You cannot educate a child who is not healthy, and you cannot keep a child healthy who is not educated."
 
The FAMS health office always strives to maintain overall wellness. Wellness does not just matter - it is critical.  It is critical not just for the student, but for teachers and staff!
 
Parents and guardians are encouraged to contact the health office with any questions or concerns throughout the year. Welcome to the FAMily.

Requirements For All 6th Grade & Transfer Students  
 
All 6th grade students MUST provide:
 
  • Recent Physical Examination (completed with in last 365 days)
  • Complete Immunization Record 
 
Tdap and Meningoccal Vaccines are both REQUIRED to begin school.
(Students may be excluded from school for failure to provide these records.) 
 
Please have your child's physician complete the "Medical Eligibility Form" for their physical. 
All medical forms can be found under the Health Concerns and Medical Forms tab.
 
*All health and immunization records can be dropped off in the main office or forwarded to [email protected]
 
Transfer Students From Out of Country :
Students transferring from a high Tuberculosis (TB) incidence country will need to be screened for TB prior to school admission.
 
  • Interferon Gamma Release Assay (IGRA) blood test 
  • Mantoux Tuberculin Skin Test (Is also acceptable)

NJ FamilyCare is federal and state funded health insurance program created to help qualified New Jersey residents of any age access to affordable health insurance. NJ FamilyCare is for people who do not have employer insurance.
